OUTREACH SPECIALIST COMMUNITY

Position Summary:

We are seeking a dedicated and compassionate Outreach Specialist to support our Enhanced Care 

Management (ECM) program. In this role, you will conduct in-person outreach within the community, 

engaging with eligible members at their residences. Your primary responsibility will be to connect with 

individuals on the eligibility list to inform them about available services, assess their needs, and help 

coordinate care and enrollment in the ECM program.

Key Responsibilities:

•  Conduct field-based outreach by visiting the homes of eligible members.
•  Engage with individuals in a respectful, professional, and culturally sensitive manner.
•  Educate members on the Enhanced Care Management program and available resources.
•  Assist members in understanding and navigating healthcare and social services.
•  Document outreach activities accurately and in a timely manner.
•  Work collaboratively with care teams, including Care Managers, Community Health Workers, and 

other support services.

•  Maintain confidentiality and adhere to HIPAA regulations.

Requirements

Qualifications:

•  High school diploma or equivalent required; some college or healthcare/social service experience 

preferred.

•  Prior experience in outreach, community health, or case management a plus.
•  Comfortable working in a field-based setting and conducting door-to-door outreach.
•  Strong communication and interpersonal skills.
•  Ability to work independently and manage time effectively.
•  Bilingual skills are highly desirable.
•  Valid driver’s license and reliable transportation required.
