Description

 

Position Summary:

The PACT Clinician provides psychosocial support as a part of a multidisciplinary team to individuals with psychiatric experiences and mental illness. The PACT clinician serves both as a primary case manager for a small caseload and provides 1:1 and group sessions to clients in other areas (wellness, vocational, family support, spirituality, social, addictions etc), mostly in a community-based setting.

This Family Clinician specializes in providing ongoing assessment and clinical services to families (parents, partners, siblings, extended family, and other important network members) in the form of family therapy, psychoeducation, mentalization based family therapy, Open Dialogue, coaching, and updates. They work directly with the Director of Family Services to provide expertise and consult to the teams for family related clinical needs and support.

Salary Range:  Min $70,000 | Mid $80,000 | Max $90,000

Qualifications Required:

  • Master’s degree in relevant field
  • Post secondary school education and training, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illness and families
  • Must be licensed or certified to work in the field, abide by a code of ethical and practice standards and be committed to a process of lifetime knowledge and skills development.
  • Licensed Social Workers (LCSW) Licensed Mental Health Counselors (LMHC) and Licensed Marriage and Family Therapists (LMFT) recommended
  • Must have strong commitment to the right and ability of each person served to live in a normal community residence, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services
  • Must have skills and competencies to establish supportive trusting relationships with persons with severe and persistent mental illnesses as well as family and other members or the clients’ network and respect for clients' rights and personal preferences in treatment.

Qualifications Desired:

  • Experience with client centered practices such as Open Dialogue, Narrative Therapy, Internal Family Systems, Motivational Interviewing, Harm Reduction, Emotionally Focused Therapy, Acceptance & Commitment Therapy, Cognitive Behavioral Therapy, Mentalization Based Approaches, and Interpersonal and Social Rhythms Therapy
  • Specialized training in family therapy approaches

Essential Functions and Responsibilities:

  • Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team (ITT).
  • Provide family clinical interventions and supports including coaching, psychoeducation, and psychotherapy
  • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT
  • Provide individual side by side support, counseling, therapy and symptom management, ensuring immediate changes are made in the treatment plan as clients’ needs change
  • Provide direct clinical services including individual, family, and group work in a community-based setting
  • Educate and support clients’ families and advocate for clients’ rights and preferences
  • Conduct holistic and comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health; use of substances or behavioral addictions; education and employment; social development and functioning; activities of daily living; and family structure and relationships
  • Act as liaison and consult with community agencies and families to maintain coordination in the treatment process
  • With other team members, participate in on-call rotation for crisis intervention/support 7days/wk, 24hrs/day.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.

Physical Demands and Work Environment:

  • Physical Demands: While performing the duties of this job, the employee is occasionally required to walk, sit, use hands to type, handle or feel objects, reach with hands and arms, bend, walk up and down stairs, talk, hear, and see. Employee must be able to occasionally lift/move up to 50 pounds
  • Work Environment: Most work is performed outside of the office setting
