JOB REQUIREMENTS

Board: Board Certified
Licenses: Licensed in WI or Universal License (IMLC)
Certifications: BLS, ACLS
Fellowship: Required
Other Qualifications: Act 10 certification acceptable

Duties & Responsibilities

Provide outpatient pain medicine services including office visits, follow -up visits, and procedures
Perform spinal cord stimulation, radio -frequency ablation, and injections
Work a flexible schedule of 2 weeks on/2 weeks off, 5 days per week, 7a -3p
Collaborate with support staff (RNs and office staff)
Maintain accurate documentation in Epic EMR
