Job Description - Palliative Advanced Practitioner

The Palliative Advanced Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive services in Palliative care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the clinical supervision of the Medical Director, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments, as appropriate, supports the development of the plan of care in consultation with the interdisciplinary team. The advanced practitioner builds from the resources of the community to plan and direct services to meet the need of individuals across care settings including patients’ homes, assisted living, long-term care, rehab facilities, clinic, and inpatient facilities. 


The territory includes all our 27 service towns from Milton down to Carver.


Schedule: Monday - Friday 8:00 am - 5:00 pm


Compensation: 62/hr. - 68/hr. Commensurate with experience


What will you be doing: 



  • Provides medical care, palliates, and manages Palliative care in compliance with the state Nurse Practice Act and Medicare guidelines.

  • Supports and establishes eligibility in collaboration with Medical Director, Liaisons, intake and internal referral process.

  • Assures that patient receives appropriate measures to control symptoms through collaboration with interdisciplinary team members.

  • Provides clinical expertise on symptom management and evaluation of terminal prognosis to patient, family and IDT.

  • Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care.

  • Consults with the patient's attending physician, the Palliative and Hospice Medical Director and Senior Advanced Practitioner of Hospice Clinical Practice and the IDT regarding  the ongoing care needs and medical management related to the patient's serious illness, condition or injury.

  • Communicates with other community health practitioners to coordinate the plan of care.

  • Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice care.

  • Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare.


 


What we’re looking for in you:



  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner or Physicians Assistant Program.

  • Current nursing licensure in State and CPR certification, as required by organizational policy.

  • Master's or doctoral degree with a minimum of one (1) year Advanced Practitioner experience who has experience in geriatrics, oncology, palliative care and/or hospice care.

  • Accreditation from an approved certifying body for advanced practice nursing as required by State.

  • Certification in a specialist area preferred; (e.g. ACHPN)

  • Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist.
