Palliative Care LPN

Full Time, Day Shift Monday - Friday Only. No On-Call or Weekends Location will be to various facilities and patient homes within the Harrisburg, West Shore, Hershey and York area. This unique role offers you the opportunity to make a significant impact in the lives of senior patients facing serious illness.You'll manage and coordinate palliative care, ensuring timely and accurate orders for testing, medications, and referrals. Communication with patients, families, and providers, along with updating treatment plans and addressing concerns, is key. Accurate documentation of health information is essential.Providing hands-on care during patient visits, including taking vital signs and assisting providers, is crucial. You'll handle medication management, coordinate referrals, therapy orders, and telemedicine appointments, manage prior authorizations, and update care plans as needed.
