Licensed Clinical Social Worker – Palliative Care & Hospice

Full-Time or Per Diem |  $45–$50/hr

We specialize in supporting seniors, especially those with dementia, through interdisciplinary care that integrates pharmaceutical expertise to improve quality of life.

What You’ll Do:

• 
  

  Psychosocial Support: Assess the emotional, social, and psychological needs of patients and caregivers

  
  


• 
  

  Care Coordination: Collaborate with physicians, nurses, and care teams to build effective care plans

  
  


• 
  

  Family Education & Advocacy: Help families access resources and navigate complex care decisions

  
  


• 
  

  Bereavement Services: Provide post-loss counseling and support to families for up to a year

  
  


• 
  

  Regulatory Compliance: Maintain accurate and timely documentation in accordance with state/federal standards

  
  

What We’re Looking For:

• 
  

  License & Credentials: Active LCSW license required; NPI and credentialed to bill for CBT

  
  


• 
  

  Experience: Preferred experience in palliative care, hospice, geriatrics, or chronic disease management (including home health)

  
  


• 
  

  Skills: Compassionate, autonomous, team-oriented, and excellent at time management and communication

  
  


• 
  

  Other Requirements: Valid driver’s license and insured vehicle for patient visits as needed
