Coverage Area: South, Centra, and North San Diego

Your Role

• 
  

  Emotional Support & Counseling: Assess psychosocial needs and provide compassionate counseling and crisis intervention.

  
  


• 
  

  Care Planning: Collaborate with nurses, physicians, and other team members to create individualized care plans.

  
  


• 
  

  Family Education & Advocacy: Help patients and families understand resources and navigate complex systems.

  
  


• 
  

  Bereavement Services: Support families with a structured bereavement program for up to 12 months after loss.

  
  


• 
  

  Compliance & Documentation: Ensure all care meets state, federal, and regulatory guidelines.

  
  

What We’re Looking For

• 
  

  Education: Master’s Degree in Social Work (MSW) required.

  
  


• 
  

  Experience: Prior experience in healthcare, hospice, or palliative care settings preferred.

  
  


• 
  

  Skills: Strong interpersonal, organizational, and communication skills. Able to work independently.
