The position is open and extended upto Mechanicsburg, Harrisburg, and surrounding areas. 

About the Role: Deliver Expert Palliative Care Services

As a Palliative Nurse Practitioner, you will expertly assess, diagnose, and manage symptoms related to chronic, complex, or life-limiting conditions. You will lead comprehensive care planning alongside patients, caregivers, and interdisciplinary healthcare professionals to enhance quality of life, facilitate goal setting, and ensure compassionate support throughout the palliative journey.

Key Responsibilities: Comprehensive Palliative Assessment & Support

• Conduct thorough physical and psychosocial assessments for adults with advanced illness.
• Develop, implement, and continually update individualized care plans to address symptoms, pain, and stress.
• Collaborate with physicians, nurses, social workers, and allied health professionals to ensure holistic, interdisciplinary care.
• Coordinate and facilitate family meetings to support advance care planning and shared decision-making.
• Provide patient and family education on disease process, care options, and symptom management strategies.
• Advocate for patient goals and preferences in alignment with evidence-based palliative nursing practice.
• Accurately and efficiently document patient encounters and plans in the electronic health record (EHR).
• Promote a culture of respect, inclusivity, and empathy within the care team and among those you serve.

Qualifications: Palliative Nurse Practitioner Experience & Compassion

• Bachelor's degree or higher with 5+ years of Clinical Experience
• Current and unrestricted Nurse Practitioner (NP) and RN license state of PA
• Minimum 1 year of clinical experience in palliative care, hospice and home health
• ANY of AHA BLS or AHA ACLS

Schedule and Shift Details: Primarily Monday – Friday 8:00am - 5:00pm
