The Participant Support Specialist (PSS) is responsible for ensuring required monthly home visits are completed for all participants receiving Shared Living services. This role ensures regulatory compliance by conducting in-person visits, documenting outcomes, and escalating concerns. The PSS also works as a Direct Support Professional (DSP) in day services from Monday through Thursday, maintaining continuity of care for participants and supporting team-based service delivery.

Essential Responsibilities:

Home Visit Oversight (Sundays):

Conduct in-home visits for assigned participants in Shared Living placements.

Ensure all visits meet Nebraska DHHS and internal compliance requirements (1 per month; 2 for advanced-level).

Document all visits in the designated system on the same day of the visit.

Identify and escalate safety, service delivery, or quality concerns according to protocol.

Track follow-up items discussed during visits and refer to the appropriate Shared Living Coordinator (SLC).

Day Services Support (Monday–Thursday):

Provide direct support to participants in day programming.

Assist with individual goals, community engagement, and skill-building activities.

Support consistent care practices aligned with person-centered plans and behavior strategies.

Compensation: $17.00 - $19.00 per hour
