Patient Advocate Coordinator

Trustbridge Hospice, a part of Empath Health, is seeking an organized and empathetic Patient Advocate Coordinator to join our team in West Palm Beach. This position is onsite.

This full-time position works Monday through Friday, 8:30 a.m – 5 p.m., supporting our admissions team and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You’ll Do:

• Provide customer service/phone etiquette, education, and support to patients/families in need of hospice support.


• Review revocations, no longer terminal, non-admit and pending referrals to capture new admissions to hospice services.


• Utilize expertise to assess needs for patients and families with concerns, questions, or emotional needs related to hospice services


• Provide compassionate education and counseling to help patients and families understand hospice philosophy, services, and care options


• Support a compassionate, patient-centered environment that reflects the mission and values of Trustbridge Hospice.

What You’ll Need:

• 2-3 years of Medical Marketing/Sale experience, preferably in a Hospice or Home Care environment.


• Case management or Social work experience highly preferred


• Knowledge of Medicare, HCFA and state regulations.


• Organized, flexible, self motivated with ability to relate to all levels of the professional medical team.


• Works well with supervision and staff as a team player.
