Job Description:

I. Position Summary

 

The primary role of the Senior Patient Care Associate is to protect, sustain, and nurture the patients. They also engage and guide peers. They assist with activities of daily living, care of the patient’s environment, and are able to develop meaningful relationships with patients and colleagues. The goal of the work of the nursing assistant is to provide support that promotes a high quality of life that is in line with the patient’s vision. The Senior PCA works as a member of a team that strives to achieve a home-like setting while maintaining excellence in all areas of care.  

 

The Senior PCA will, in addition to the above, take a leadership role on the health care team and will take responsibility for mentoring and coaching peers and co-workers. The Senior PCA will give input into the development of the care plan, will read medical records and be able to synthesize the key themes, will communicate directly with the health care team through documentation in the medical record and emails, and will contribute to fall prevention and Quality Improvement projects.

III. Position Responsibilities:

Typical PCA responsibilities:

1.      Recognize and respond to the needs of the patients and families

2.      Recognize and communicate changes in the patient’s condition to the appropriate nursing team member

3.      Adhere to Safe Patient Handling practices, i.e. understand and safely use lift equipment and tools to promote safe patient handling practices such as sliding sheets and hover mats.

4.      Participate in Quality Improvement programs

5.      Creates home-like experience for the patient that supports their individual preferences

6.      Assist patients with all aspects of personal care while encouraging independence and choice

7.      Document pertinent information regarding care delivered in the designated areas of the patient/patient’s electronic medical record.

8.       Report accidents and incidents as they occur and offer plans to correct/prevent event again.

9.      Report any observed or suspected patient abuse immediately

10.  Observe and report on skin integrity

11.  Participate in daily team reports/huddles and care planning meetings

12.  Be proficient in obtaining vital signs and other procedures such as temperature, pulse, respirations, blood pressure, weight, , pulse oximetry and urine and stool specimens as directed

13.  Follow infection control practices

14.  Maintain confidentiality and privacy of the patient according to HIPAA regulations

15.  Understand patient’s individual preferences for engagement and activities and encourage that engagement on the unit

16.  Maintain cleanliness and organization of the patient’s room and common program areas taking into consideration patient preferences Prepare, serve, and clean up after meals and snacks.

IV. Qualifications for PCA:
  • Fluent English skills required – verbal and written

  • Competency with basic computer skills such as corresponding with colleagues via email, ability to create and attach word documents to emails, and proficiency with navigating the HSL HUB to access information.

  • Flexibility and ability to work independently and collaboratively
