The Home Health Patient Coordinator is a vital member of our care team, ensuring the efficient coordination of patient services from referral through ongoing care. This role serves as a central point of communication between patients, caregivers, healthcare providers, and clinicians while helping maintain a seamless patient experience.

Key Responsibilities

• Coordinate patient scheduling and clinician assignments to ensure timely care delivery.


• Process new referrals and support patient admissions and onboarding activities.


• Communicate professionally with patients, caregivers, physicians, hospitals, and community partners.


• Manage referral documentation, medical records, and data entry with accuracy and attention to detail.


• Support care coordination efforts by facilitating communication between clinical staff and referral sources.


• Answer incoming calls and respond to inquiries while maintaining HIPAA compliance and patient confidentiality.


• Assist with obtaining and tracking physician orders, insurance information, and other required documentation.


• Collaborate with the clinical team to promote efficient operations and positive patient outcomes.

Qualifications

• Experience in healthcare administration, patient intake, care coordination, scheduling, or a medical office setting preferred.


• LPN, Medical Assistant, CNA, or similar healthcare background strongly encouraged.


• Experience with insurance authorizations or healthcare referrals is a plus.


• Strong organizational and multitasking abilities with the capacity to manage competing priorities.


• Excellent communication and customer service skills.


• Working knowledge of medical terminology, anatomy, and physiology preferred.


• Proficiency in Microsoft Office applications, including Word, Outlook, and Excel.


• Ability to work onsite at our Appleton office Monday through Friday, 8:00 AM to 5:00 PM.
