Patient Care Coordinator

• Coordinates continuity of patients following their hospital discharge, post-discharge follow-up appointment and follow-up with their primary care provider and specialist. 


• Prepares the patient for their telemedicine visit by assisting patients/caregivers with joining the telemedicine link. 

• Invites the language interpreter to telemedicine visit, as needed.  


• Completes intake of the patient’s medical history and documents their vital signs. 

• Maintain the flow of the clinical schedule, booking, rescheduling, blocking, and cancelling appointments as deemed appropriate.  

• Reconciles allergies and medications, and reviews preferred pharmacy and primary care provider information with the patient/caregiver.  

• Pre-register and check out patients 

• Able to communicate effectively and maintain patient records, files, and reports.  

• Faxing documents, orders, and results to the appropriate destination.  

• Self-disciplined, energetic, passionate, innovative, adaptable, and professional  

• Proficient in Word, PowerPoint and Excel. 

• Secure and ensure confidentiality of sensitive information.  

• Inform patient/caregiver of stable/benign results annotated by clinicians.  

Position Requirements:  

Formal Education: This position requires a minimum formal education of a High School Diploma or equivalent and a minimum of 2-year job-related experience.  

Experience: Desired (not required): Experience using telemedicine platforms, scheduling appointments, and completing pre-registration.  

License, registration, or cert. required: Medical Assistant Certification , Spanish preferred.
