Job Description - Patient Care Coordinator

Job Summary

Our client is seeking a compassionate, organized, and patient -focused Patient Care & Office Coordinator to support both clinical care and daily office operations in a growing Direct Primary Care (DPC) practice. This role combines patient care, scheduling, administrative coordination, and patient engagement to help deliver an exceptional healthcare experience. 

Schedule: 24–30 hours per week | Flexible Schedule

Key Responsibilities

Patient Care & Clinical Support

  • Greet patients and visitors professionally
  • Perform blood draws and administer vaccines (training available)
  • Assist with CLIA -waived testing
  • Manage lab workflows and inventory

Patient Communication & Experience

  • Answer phones and respond to patient inquiries
  • Onboard new patients and follow up with prospective patients

Scheduling & Care Coordination

  • Schedule appointments, referrals, imaging, and consultations
  • Request and track outside medical records
  • Coordinate prior authorizations as needed

Administrative & Office Operations

  • Maintain accurate EHR documentation
  • Process invoices and assist with billing activities
  • Manage office supplies, deliveries, mail, and light office upkeep

Compliance & Safety

  • Participate in OSHA, HIPAA, and CLIA training
  • Perform routine safety and equipment checks as trained

Community & Practice Growth

  • Support social media initiatives and community events


Requirements

Required Qualifications

  • 2+ years of clinical experience (MA, LPN, RN, or similar)
  • Strong customer service and communication skills
  • Excellent organizational and computer skills
  • Professional, dependable, and patient -centered approach
  • High school diploma or equivalent
  • Must be 18 years or older

Preferred Qualifications

  • Phlebotomy experience or willingness to learn
  • Administrative, marketing, or outreach experience
  • Bilingual English/Spanish
  • Previous customer service experience
