Patient Care Coordinator

Hours: 37.5 hours/week
Position Summary:
The Patient Care Coordinator (PCC) is a non-medical role that provides compassionate support to patients during and after their pregnancy test appointment. Primary responsibilities include serving as a Patient Advocate by providing unbiased education on pregnancy options, discussing sexual risk avoidance, offering community referrals, and providing emotional and spiritual support as appropriate. The PCC also meets one-on-one with patients to assist with state insurance and community program applications and helps connect them with the resources, education, and support needed to make informed decisions and navigate their pregnancy journey with confidence and care.
The Patient Care Coordinator reports to the Patient Care Manager. There will be a yearly written and oral evaluation as well as regular one-on-one meetings.
Qualifications:
Bilingual (English and Spanish) preferred but not required.
Committed Christian who demonstrates a personal relationship with Jesus Christ as Lord and Savior and is accountable to a local Bible-believing church.
Agreement with Choices’ Statement of Principle, Statement of Faith, Mission Statement, and policies of the organization, and is committed to the Ministry of Choices.
Strong commitment to the sanctity of human life and biblical sexual ethics and have a sincere desire to reach and help at-risk patients considering abortion.
Proven management skills including, problem-solving, clear communication, excellent administrative skills, the capacity for motivation and able to provide constructive feedback.
Effectively manage assigned projects, carry out responsibilities with minimal supervision and complete tasks as required.
Responsibilities:
- Meet regularly with patients during urine pregnancy test appointments to provide clear, compassionate, and nonjudgmental education about pregnancy options, including abortion, adoption, and parenting and encouraging spiritual growth and goal setting when appropriate.
- Maintain and regularly update referral and resource materials to ensure information is current, accurate, and appropriate for Choices’ staff, clients, and volunteers. The PCC will act as a liaison between Choices and other outside agencies to create easy pathways for referrals and access to brochures, program information, and other materials.
- Provide administrative support such as copying, charting, auditing, and ensuring incoming phone calls and text messages are addressed promptly by providing support and coverage when the receptionist team is unavailable or experiencing high call volume.
- Provide support in the recruitment, interviewing, onboarding, training, support, and education of non-medical volunteers as needed.
- Shared team duties; participate in the organization/tidiness and cleanliness of the reception, lobby, bathrooms, lab, volunteer room, etc.
- Coordinate and monitor patient follow-up processes by assigning follow-up responsibilities, tracking completion, and ensuring patients receive consistent and timely care.
- Proactively manage assigned projects.
General Staff Duties:
As requested by the employee’s supervisor, Client Services Director, or CEO:
- Attend staff meetings and other required staff gatherings.
- Participate in occasional travel for training, conferences, or other work-related purposes as needed.
- Attend and assist with Choices events and other organizational activities as needed, including Sanctity of Human Life (SOHL) events and the annual Baby Shower.
- Participate in opportunities to represent Choices with churches, community organizations, and other partners.
- Take an active role in Choices’ outreach, development, and network-building efforts, including participating in and assisting with the annual Choices Fundraising Banquet, Walk for Life, and other organizational events as requested.
- Commit to ongoing learning and professional development, including staying informed about life issues and matters relevant to the employee’s role.
