Position Summary:

The Patient Care Manager will support the Head of Patient Care in managing the daily operations of remote diabetes education and management services. This role is focused on coordinating patient care, supervising and developing staff, and maintaining high-quality standards across our telehealth and remote monitoring programs. The Patient Care Manager will play a vital role in ensuring patients receive timely, effective support while driving operational excellence.

Key Responsibilities:

• Manage daily workflows for telemedicine and remote patient monitoring programs.
• Supervise, mentor, and support diabetes educators and medical assistants.
• Conduct virtual assessments and collaborate with the Head of Patient Care to develop individualized care plans.
• Provide patient education and counseling on diabetes self-management, including nutrition, physical activity, medication adherence, and lifestyle modifications.
• Monitor and analyze patient health data from remote devices to coordinate timely interventions.
• Track program performance, patient outcomes, and satisfaction, recommending improvements where needed.
• Ensure compliance with HIPAA, ADA, AADE, and other relevant standards and regulations.
• Collaborate with leadership to implement quality improvement initiatives and adopt new digital health technologies that enhance patient support.

Qualifications:

• Certified Diabetes Care and Education Specialist (CDCES) credential required.
• Nursing degree required.
• 3–5 years of experience in diabetes education, patient care, or healthcare management.
• Strong leadership, organizational, and communication skills.
• Experience in telehealth or remote patient monitoring is a plus.
• Bachelor’s degree in healthcare, nursing, or a related field preferred.
