We are seeking a Patient Care Manager (RN) to join our dedicated team in Ormond Beach, FL. In this leadership role, you'll oversee patient care delivery, lead a team of healthcare professionals, and ensure top-tier clinical outcomes in a supportive and collaborative environment.

Key Responsibilities

• Develop, implement, and evaluate individualized care plans for home health patients

• Coordinate day-to-day clinical operations to ensure high-quality care and service delivery

• Supervise, mentor, and support nursing and allied health staff

• Conduct regular in-home patient assessments and document clinical findings

• Collaborate with physicians, therapists, and case managers to ensure continuity of care

• Maintain compliance with all state, federal, and accreditation standards

• Educate patients and families on diagnoses, treatment plans, and long-term care strategies

• Utilize electronic health records (EHR) systems for accurate and timely documentation

Qualifications

• Current Registered Nurse (RN) license in Florida required

• BSN preferred; ASN with strong leadership experience will be considered

• 2+ years of clinical nursing experience, with at least 1 year in home health or community-based care

• Minimum of 2 years of supervisory or management experience in a home care setting preferred

• Excellent organizational, interpersonal, and communication skills

• Proficient with EHR platforms and general computer applications
