JOB TITLE:           Patient Intake/Care Coordinator



STATUS:             Regular Full-Time



HOURS:                 Hours are between 7:30am – 5pm, Monday – Thursday and 8am-12pm Friday



We are seeking a motivated and organized Intake and Care Coordinator to join our team. This hybrid role combines responsibilities of both care coordination and patient intake, ensuring a seamless experience for our patients from their initial contact through ongoing care. The ideal candidate will have a strong background in healthcare coordination, excellent communication skills, and a commitment to enhancing patient satisfaction.


 


Flexibility during clinic hours is necessary to align with the needs of patients and position workflows.  Shifts are typically up to 9 hours, longer shifts may be required at times.  Availability to work this schedule is a requirement of the position.

KEY RESPONSIBILITIES: (Job Description Available Upon Demand)




  • Supports and implements the practice mission and strategic vision within their respective departments.

  • Handles confidential information requiring professional discretion and compliance with protected health information, data integrity and security policies.

  • Serve as the primary point of contact for incoming patient referrals and inquiries, responding timely to patient inquiries.

  • Monitoring multiple systems to track patient inquiries including phone, email, text, and social media.

  • Collect and verify patient information, including medical histories and insurance details, to facilitate the admission process.

  • Coordinate with referring providers to obtain necessary documentation and ensure a smooth transition into our services.

  • Coordinate with community providers to schedule administrative meetings with Alivation's management team to improve the referral processes.

  • Educate patients and their families about Alivation Health's services, addressing any questions or concerns.

  • Follow Alivation guidelines, which may change from time to time, to assign patients to the correct new patient provider.

  • Track the personalized care plans prescribed by healthcare professionals for patient compliance, including but not limited to, scheduled visits, integrated care coordinated visits, follow through on referred services.

  • Monitor patient compliance and inform correct individuals as necessary, addressing any emerging needs or challenges.

  • Assists in maintaining accurate Electronic Health Records.

  • Other clinical or administrative duties as assigned to meet practice needs.


 


Additional Responsibilities:


 



  • When responsibilities within the position are completed, additional tasks will be assigned in Nursing, Treatment Technician, or Reception depending on applications skill sets.  This position may float to other roles at the direction of the manager depending on patient and clinic needs.

QUALIFICATIONS:  High School Diploma | College Degree Preferred | Familiar with EMR & PM System | Medical Terminology | Attendance Is Essential Of This Position | Proficient in Microsoft Platforms 



EXPERIENCE:  Previous Medical Clinic Experience Preferred
