Patient Navigator

The Patient Navigator plays a crucial role in enhancing the patient experience at Desert Parkway Behavioral Healthcare Hospital by ensuring smooth transitions between inpatient and outpatient care. This professional is dedicated to supporting patients in navigating their mental health journey, linking them to appropriate resources and ensuring that they receive the best possible care.

Key Responsibilities:

• Collaborate with the inpatient treatment team to coordinate discharge planning and ensure continuity of care.
• Educate patients and their families about outpatient services and available community resources.
• Address barriers preventing patient referrals to outpatient services and facilitate effective communication between all parties involved.
• Conduct assessments and 1:1 visits with patients to gauge their needs and eligibility for outpatient programs.
• Track and analyze data related to patient transitions from inpatient to outpatient care.
• Maintain timely and accurate clinical documentation in compliance with healthcare standards.
• Coordinate with various departments such as Intake and outpatient program leads for seamless patient transitions.

• Bachelor's degree in Social Work or related field required, Master's degree preferred.
• Previous experience in case management, social services, or behavioral health required.
• Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
• Ability to problem-solve and develop care plans in collaboration with clinical staff.
