JOB SUMMARY: The Patient Navigator will play a critical role in guiding patients through their healthcare journey. The primary responsibilities will include assisting patients with check-in and check-out processes, scheduling and managing appointments, and providing support with insurance eligibility and applications. The Patient Navigator will serve as a key point of contact for patients, ensuring they understand and complete necessary paperwork and applications. The goal is to facilitate a smooth and supportive experience for patients from their initial visit to their follow-up care. The Patient Navigator is also expected to actively participate in, and support quality improvement activities and the clinical practice team efforts emphasize the importance of the individual patient and putting the patient’s needs first.

Good reading eyesight and vision for close work. This role involves working in varying weather conditions, both Summer and Winter.
