Job Description - Patient Success Coordinator

The Patient Success Coordinator is a key member of the SynergenX team, dedicated to ensuring patients feel supported, educated, and engaged throughout their care journey. This role combines direct patient interaction with cross-team collaboration to maximize patient satisfaction, compliance, and retention. 

The Coordinator serves as a primary contact for new self-inject patients, providing follow-up education after their first visit, setting expectations, addressing concerns, and ensuring patients have the tools and confidence they need to be successful. In addition, this role supports patient retention by reaching out to patients who miss appointments, assisting with rescheduling, and being readily available for questions or assistance. 

Beyond direct patient calls, the Coordinator partners with the Call Center and Operations teams to contribute to broader patient engagement and retention initiatives, ensuring consistent communication and an exceptional patient experience across all touchpoints.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Two weeks of paid on-site training.

Responsibilities:

  • Conduct initial and follow-up calls with new patients to reinforce education, review expectations, and build patient confidence in their care plan. 

  • Provide a consistent and supportive point of contact to answer questions, resolve concerns, and enhance the patient experience. 

  • Reach out to patients who miss or cancel appointments to reschedule and encourage ongoing engagement in their therapy. 

  • Track and document patient interactions, concerns, and follow-up actions within the EMR system to ensure continuity of care. 

  • Collaborate closely with the Call Center team to support patient outreach campaigns, proactive engagement efforts, and service recovery initiatives. 

  • Partner with Operations teams to align patient success efforts with company-wide strategies for retention, satisfaction, and growth. 

  • Participate in ongoing patient experience initiatives, sharing insights and feedback to improve processes and enhance care delivery. 

  • Monitor patient feedback and trends, escalating issues when needed to ensure quality of care and patient well-being. 

  • Serve as an advocate for the patient, ensuring they feel supported, informed, and valued throughout their journey with SynergenX. 

  • Other duties may be assigned.  

Qualifications:

  • High School Diploma or GED
  • 1+ year of Customer Service experience
  • Experience with CRM or EHR software preferred 
  • Strong communication skills, attention to detail, and ability to multitask
  • Professional demeanor, positive attitude, and good judgment
