KellyConnect® is seeking a compassionate and customer-oriented Patient Support Specialist to join our team at a premier employer in Horsham, PA. As a Patient Support Specialist, you will play a critical role in managing the day-to-day operations of the patient support program, assisting terminally ill patients and their caregivers with eligibility, enrollment, travel logistics, and reimbursement. This is a unique opportunity to directly impact the health and well-being of others while working in a dynamic and forward-thinking environment. 

Job Specifications:
- Job Type: Full-time
- Company: KellyConnect®
- Location: Horsham, PA (Hybrid option)
- Pay Rate: $35 per hour
- Shift: Rotating with availability between 8AM to 8PM, Monday through Friday

Roles and Responsibilities:
- Serve as the point person for a select number of patients/caregivers, supporting their treatment through an online case management system.
- Respond to inquiries from patients/caregivers/sites regarding the patient support service offerings using a call guide resource.
- Perform administrative functions such as requesting and responding to travel and logistics, ensuring prompt and supportive assistance to patients/caregivers.
- Validate patient travel expenditures in compliance with SOPs and provide reimbursement through third-party supplier partner.
- Enter and maintain accurate data and records into the patient management tool in compliance with program requirements.
- Follow all SOPs to ensure program compliance and capture necessary data for the program.
- Proactively work with patients/caregivers, showing empathy and compassion throughout their treatment plan.
- Monitor performance and identify operational improvements in the end-to-end patient experience to enhance service offerings.
- Perform other duties as assigned.

Qualifications & Skills:
- Associate's degree required (B.S. or B.A. degree preferred).
- Minimum of 3 years of relevant experience  (Background in: medical, case management, mental health, hospice, critical care, social work, life insurance)
- Excellent oral and written communication skills with the ability to demonstrate patient empathy and support.
- Bilingual capabilities are a plus.
- Ability to learn and work within numerous IT platforms to document patient cases.
- Strong problem-solving skills and ability to work through ambiguous situations to ensure a positive customer experience.
- Excellent care management and record-keeping abilities in accordance with program design and compliance standards.
- Willingness to take ownership, be proactive, and support patients/caregivers throughout their treatment journey.
- Ability to work in a fast-paced environment with changing patient/caregiver needs.
- Strong organizational skills and ability to prioritize activities to address patient travel, logistics, and reimbursement support needs.
- Comfortable working in a team environment and building partnerships across internal and external organizations.
- Passionate about supporting terminally ill patients and translating their needs into effective services.
