About the Role:

Join Tradition Home Health Care as a Home Health Aide (HHA-PCA) and make a meaningful difference in the lives of our clients in Brooklyn, NY. We are looking for compassionate and dedicated individuals to provide essential support and care to those in need.

Responsibilities:

Assist clients with daily living activities, including bathing, dressing, and grooming.

Provide companionship and emotional support to enhance clients' quality of life.

Administer medication and monitor clients' health as directed by healthcare professionals.

Help with meal preparation and nutritional guidance tailored to clients' needs.

Maintain a clean and safe environment for clients.

Document care provided and report any changes in clients' conditions to supervisors.

Support clients with mobility and transportation to appointments or social activities.

Collaborate with a team of healthcare professionals to ensure comprehensive care.

Requirements:

Valid Home Health Aide (HHA) or Personal Care Aide (PCA) certification.

Previous experience in home care or healthcare settings is preferred.

Strong communication skills and a compassionate demeanor.

Ability to follow care plans and instructions from healthcare professionals.

Reliable transportation to travel between client homes.

Background check clearance and current CPR/First Aid certification.

Willingness to work flexible hours, including weekends and holidays.

Must be physically able to assist clients with mobility and personal care tasks.
