PCHH Director & Case Manager

Responsibilities (including, but not limited to:) 

• Provides leadership for the implementation and coordination of health home activities


• Coordinates activities of other health home staff


• Champions practice transformation based on health home principles


• Monitors health home performance and leads improvement efforts


• Training and technical assistance


• Data management and reporting


• Assist with enrollment/discharge processing

Education/Experience Requirement

• Current RN license recognized by the State of Missouri Board of Nursing or Nurse Practitioner or MD/DO license


• Strong experience with clinical application software


• Knowledge of care in accordance with regulations and practices established by license board


• Consistent demonstration of a team approach to care


• Requires extensive knowledge of the PCHH program.


• Computer experience in database management, word processing, and spreadsheets required.

Work Location:
This position is split between our Fordland and Kimberling City clinic locations. There is flexibility in both the weekly schedule and the number of hours worked at each site, based on clinic needs and candidate availability.

Work hours: Full-time, hourly position - In order to maintain full-time status, a minimum of 30 hours per week on average are required, though 40 hours a week will be expected for this position. Hours are subject to change. This position can choose between 8-hour shifts 5 days a week or 10-hour shifts 4 days a week.  Clinic is open 8:00 am to 6:00 pm Monday through Friday and 8:00 am to 4:00 pm on Saturdays.
