Pediatric Hospital to Home Transition Nurse (Night Shift) - RN

Shift: Full time night shift 4-5 nights per week

Pay Range: $38-$45

We are seeking a compassionate and skilled Registered Nurse (RN) to support the transition of pediatric patients from the hospital setting to home care. This full-time night shift position involves providing direct patient care, training and supporting families in the home environment, and ensuring a smooth and safe transition process.

 Key Responsibilities:

•              Provide skilled nursing care to pediatric patients in their home during the initial transition from hospital to home.

•              Educate and support the patient’s family/caregivers in managing the child’s medical needs and equipment.

•              Administer medications and treatments as prescribed.

•              Operate and troubleshoot home medical equipment including ventilators, tracheostomy care, and gastrostomy (G-tube) feedings.

•              Monitor the patient’s condition and respond to changes or emergencies appropriately.

•              Maintain accurate and up-to-date clinical documentation in EMR system.

Qualifications:

• 
  Current and valid RN license in the state of practice (required)


• Minimum of 1 year of pediatric nursing experience preferred


• 
  Experience with tracheostomy care, ventilators, G-tubes, and medication administration in the home or hospital setting (required)


• Ability to work independently and manage complex pediatric cases


• Strong communication, organizational, and family education skills


• BLS certification (PALS preferred)
