Job Description - Peer Navigator

Position Summary:


The Sage Way Peer Navigators are Certified Intentional Peer Support Specialist and provide peer support through Meaningful Activities as well as though provision of educational activities focused on goal planning, self-management, problem solving skills and Vocational Preparedness. Navigators assist participants other supportive services such as housing, food, health care, and wellness; and
provide information, trainings, and technical assistance to the community, law enforcement, and providers
 



Duties and Responsibilities:


Program/Project Management Support



  • Provide outreach services to locate and connect unduplicated individuals, quarterly.  



  • Offer linkage(s) to community resources, treatment, and wellness programs, as identified and requested by the Participant, which must include but is not limited to:   



  • Treatment for mental health and/or substance use, including Medication Assisted Treatment (MAT), as applicable.  



  • Detox and/or rehabilitation services.  



  • Overdose Prevention Through Intensive Outreach Naloxone and Safety (OPTIONS); 



  • Medical care.  



  • General wellness and/or Recovery services.  



  • and Legal aid services, as applicable.  



  • Support Participants in navigating challenges to access services, as needed and appropriate. 



  • Provide support and services in alignment with multiple recovery modalities.   



  • Provide Participants with education and information about community resources, including treatment and Primary, Secondary and Tertiary Prevention services.  



  • Provide follow-up contact with Participants within thirty (30) calendar days of the initial contact, as able and consented to by the Participant.   



  • Provide support to Participants with accessing treatment and/or Recovery support(s).  



  • If the Participant indicates they no longer want to continue services with a current community provider, the Peer Navigator shall support the Participant in identifying other Participant directed options.  



  • If the Participant indicates they no longer are interested in working with a new community provider, the Peer Navigator shall continue to provide Peer Support and outreach as desired by the Participant.  


Information Preparation



  • Promote and disseminate a calendar of services offered including but not limited to Peer Lead Support groups, Community and Stakeholder Trainings, and other related activities. 



  • Collaborate with Prevention staff to create and disseminate overdose prevention information and materials.  



  • Promote and disseminate HIV/ HCV and other sexual health information.  



  • Promote and refer participants to other clinical services including but not limited to: Case Management; Wellness Programs; Maternal Child Health, General Medical Support, and other available services within the catchment area.  



  • Ensure that the website has current and accurate information about the program 



  • Ensure the Wabanaki Careline has the correct program information for referrals.  


Logistical and Event Coordination



  • Assist and collaborate with health fair and other outreach events. 

  • Plan and implement special events as appropriate. 



  • Perform other duties as assigned.



Education and Experience Required:



  • CIPSS Certified or eligible  

  • Certified Recovery Coach or eligible

  • 2 years of providing Substance use and Mental Health Related Services

  • Familiar with multiple paths to recovery

  • Lived experience preferred / one or more years of recovery work 


  



    Skills and Qualifications Required:



    • Excellent planning, time-management, analytical, and organizational skills  

    • Proficient in Microsoft Office, including Outlook, Word, Excel, PowerPoint as well as Microsoft 365, Teams, and Zoom. Project Management software experience is preferred

    • Possession of a valid driver’s license and the ability to be insured under the Agency’s automobile insurance plan at the standard rate 


    • Must pass criminal background check
