This position will work in our Residential program at our Rush-Scottsville Road home in Rush and our East River Road home in West Henrietta (the houses are 3 minutes apart)

Duties and Responsibilities

• 
  

  Demonstrates sensitivity and the respect for the cultural diversity of others

  Follow the responsibilities, as necessary, of a mandated reporter

  Assume responsibilities of the Residential Nurse when the regular Residential Nurse is on leave, as scheduled.

  Physically examines and determines health status for all residents weekly and as needed.  Develops a rapport with each resident living in the houses assigned to support by interacting regularly with each resident in their home environment.

  
  


• 
  

  Accurately and consistently completes all required documentation practices, in a contemporaneous manner, utilizing the PrecisionCare software and in written form, when necessary.

  
  


• 
  

  When filling in for a regular Residential Nurse, reviews on-call nursing notes entered into PrecisionCare on a daily basis and follows up when necessary.  Communicates new and/or pressing medical/health/illness information to the on-call RN via email.

  
  


• 
  

  Develops and maintains appropriate written general and resident-specific Plans of Nursing Services (PONS) as dictated by OPWDD regulations, as requested.

  
  


• 
  

  Assess (initially and annually) each resident’s ability to self medicate in the form and format required by OPWDD.  Subsequently develops appropriate objectives in this area according to skill level, as requested.

  
  


• 
  

  Develops and updates specific medication information, as needed.

  
  


• 
  

  Assist with intake preparations for individuals participating in medical respite at State Road, as requested by the State Road Residential Nurse.

  
  


• 
  

  May be requested to accompany residents to appointments, as needed. 

  
  


• 
  

  Reviews and records monthly doctors’ orders, reviews and prepares monthly MAR sheets including controlled drug sheets, home med sheets and key transfer, as requested.

  
  


• 
  

  Administers medically prescribed medications to the residents, as requested.

  
  


• 
  

  Assists in overseeing AMAPs in medication administration responsibilities.  Assists in providing medication administration certification and re-certification to all AMAPs. 

  
  


• 
  

  In conjunction with the Residence Manager, follows up on all medication errors as dictated by Agency policy.

  
  


• 
  

  In conjunction with the Residential Nurse, ensure that all medical, health and safety procedures are implemented as written.

  
  


• 
  

  Places and reads staff and resident PPD tests as scheduled.

  
  


• 
  

  Reviews incident reports and provides necessary medical attention and follow-up to injuries, as requested.

  
  


• 
  

  Other duties may be assigned.

  
  

Qualifications, Experience and Training

• A valid NYS license with an acceptable driving record according to MCCC guidelines is required.


• Associate's degree from a two-year college or university in Nursing is require.  Bachelor’s degree (B.S.) from a four-year college or university in Nursing is preferred.  Certified by New York State as a Registered Nurse.  One to two years related experience and/or training is preferred.  Experience with individuals with developmental disabilities is desired.


• Must maintain New York State Registered Nurse Licensure, Agency-Approved Driver status, certification in CPR, First Aid, SCIP-r and all other training mandated by OPWDD regulations.  Must obtain OPWDD Nursing Certification.


• Must have the ability and temperament to work effectively with youth and young adults who have developmental disabilities.  Must be able to work alone and in a team-oriented environment, possess strong communication skills including communicating effectively amongst staff, exhibit patience, be an effective trainer, etc. 


• Ability to read and interpret documents such as safety rules, protocol and procedure manuals.  Ability to write routine reports and correspondence.  Ability to define problems, collect data, establish facts, and draw valid conclusions.  Ability to speak effectively before groups of people.  Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.

The work environment is primarily in a residential setting.  While performing the duties of this job, the employee may be frequently exposed to individuals who engage in aggressive, destructive and attention-seeking behaviors that may involve exposure to body fluids.  The environment is climate controlled. The employee is required to participate in travel and appointment activities; therefore, exposure to various outside weather conditions exists.  The noise level in the work environment is usually moderate.  While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts.

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk and hear.  The employee frequently is required to sit, stand, walk, stoop, kneel, crouch, or crawl.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.  The physical demands of the job will vary from site to site.  Staff may need to assist with lifting and transferring of individuals and/or items requiring some lifting and carrying.  The employee must regularly lift and/or move up to 50 pounds repeatedly and have the strength to resist force for extended periods of time.  Mary Cariola Children’s Center uses SCIP-r approved physical interventions as part of some individuals behavioral support plans.  Staff will need to be able to sit on their knees for up to 20 minutes in both indoor and outdoor settings.
