Perinatal Care Coordinator

Position Summary: 

The American Indian Perinatal Coordinator works to provide care-coordination support and culturally grounded perinatal education to American Indian (AI) families throughout the pre and postnatal process. The Perinatal coordinator is responsible for providing the knowledge, skills and resources parents need to provide a safe, stable environment that supports a healthy pregnancy, labor/delivery and postnatal resources through a culturally based lens.

Essential Duties and Responsibilities: 

Responsibilities

• Prenatal care: Help      patients access care, provide monthly women’s education group, and follow      up with patients, referrals as necessary 
• Case management: Assist      with prenatal patient case management, including assessments, referrals,     and program eligibility 
• Family support: Provide      indirect support to families and caregivers, including referrals for      American Indian parenting services 
• Community outreach: Participate      in community events, collect data, and advocate for families 
• Collaboration: Work      with medical providers, service providers, and American Indian community      partners 

Specific Tasks/Duties Include:

• Works as a team member of the perinatal health team
• Participates in month ICERP meetings
• Completes special projects related to program
• Provides perinatal outreach work as needed
• Maintains easily accessible tracking system for client follow-up
• Assists clients in enrollment of OB services
• Collaborates with team as needed
• Attend Locally and national Perinatal/Indigenous birth work conference
• Works collaboratively in development, evaluation, and provision of perinatal health education services
• Exhibits independence and initiative in the development and implementation of perinatal group
• Implement perinatal programs and liaisons with community agencies and resources
• Counsels and educates pregnant and postpartum clients on pregnancy, childbirth, and postpartum care
• Supports Individualized Care Plan (ICP) for each OB (includes pregnancy and postpartum care)
• Assess and document nutrition/psychosocial/health education needs for referral
• Identifies and records strengths, needs and high-risk condition on the ICP

• Demonstrates knowledge of compliance policies and procedures within the community clinic environment.
• Always maintains confidentiality by complying with HIPAA policies.
• Good communication and interviewing skills.
• Strong interpersonal skills. Ability to be sensitive with people of various social, cultural, economic and educational backgrounds.
• Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint.
• Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
• Strong written and verbal communication skills.
• Ability to work independently and as part of a team.
• Good judgment, problem solving and decision-making skills.
• 2-5 years of work experience in the American Indian Community, Knowledge of American Indian culture and local resources.

Education/Experience: 

• Preferred 2-5 years in perinatal care and or/ education
• Experience in group education and teaching
• Experience in program development
