The Perinatal Navigator will coordinate with a multi-disciplinary provider team in the delivery of comprehensive perinatal services. Responsible for navigating and supporting patients through their obstetric care with focus on several components: psychosocial, nutrition, health education, client orientation, initial assessment, individualized care plan including intervention and reassessments, and postpartum assessment and care. The goal is to improve perinatal access to care and pregnancy outcomes through enhanced prenatal care.
Location: This position will work at our Rob Road, Patterson, and Paradise sites. Schedule: Monday – Friday, from 8:00am – 5:00pm. Compensation: $24.18 - $27.06 an hour

Essential Duties and Responsibilities
Completes and maintains required documentation and tracking of health education activities for program, contractual, internal and external review and reporting, and billing needs.
Demonstrate patient advocacy by communicating with providers, ensuring the patients receive care and resources they need, and navigating their pregnancy and postpartum needs.
Attend and actively participate in internal and external trainings to maintain skills and knowledge.
Ensure that care is provided in a manner that is culturally and linguistically appropriate.
Supports and implements the organization’s vision, mission, and values.
Performs all job functions in a professional, courteous and timely manner.
Provide basic clinical services such as vital signs, point-of-care testing, and other skills within the employee’s scope of practice for this position, with adequate training and annual competency.
Fosters and promotes a culture of service excellence and self-accountability.
Responsible for rendering of the CPS Program including enrollment eligibility, nutrition psychosocial and health education assessments, interventions, and perinatal education with basic obstetrical care.
Responsible for ensuring the integrity and quality of the health services delivered in comprehensive prenatal services and in compliance with federal, state, and county standards.
Works cohesively with pediatric and dental departments to ensure patient and newborn care coordination.
Provides follow-up and completes proper documentation of post-partum delivery and lactation education.
Participates in continuous quality improvement plans for site and program.
Maintains all compliance requirements.
Provide administrative and clinical support as needed.
Performs all other duties as assigned.
Physical Demands
Lift up to 40 pounds and push up to 100 pounds (on wheels).
Hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
Vision, with or without lenses adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
High manual dexterity.
Able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning.
Education/Experience Requirements
Minimum Qualifications:
Adheres to company policies and procedures in the performance of work duties.

Bilingual in English and Spanish preferred
Maintains and promotes a safe work environment.
Demonstrates good communication skills, both oral and written.
Demonstrates proficiency in basic computer applications.

Performs work in an honest and ethical manner.
Must be willing and able to work at all locations, and during all business hours, as needed to meet patient care needs.
Valid California driver’s license, vehicle insurance, acceptable driving record and reliable transportation.
Education/Experience:
High school diploma or equivalent required.
One (1) year of experience in an OB setting required.
CPSP Certification, possession of, or attainment with 30-days of hire in position.
Medical Assistant Certificate from an accredited program.
