Neighborhood Health Association – Toledo, OH 


Position Details: 



  • Job Type: Full-time, Non-Exempt (Grant-Funded Position; continuation contingent on funding and employee competency) 



  • Schedule: Monday – Friday; no evenings, weekends, or holidays 



  • Location: Toledo, Ohio (with extensive local travel within Lucas County) 


 


Position Summary: 


Neighborhood Health Association (NHA) is seeking a full-time Perinatal Outreach Worker to join our Women's Health Outreach team. The Perinatal Outreach Worker serves as a trained community advocate, working under general supervision to empower pregnant and postpartum women to access healthcare and essential community resources. This position focuses on education, outreach, home visits, and referrals to reduce health risks and improve maternal and infant health outcomes within Lucas County. 


This role is integral to supporting high-risk pregnant women through individualized care planning, health education, and navigation of social and healthcare systems. 


 


Key Responsibilities: 



  • Identify, recruit, and enroll high-risk pregnant women in Lucas County, maintaining an active caseload of 35-40 clients in alignment with grant guidelines. 



  • Develop and implement individualized care plans based on client needs, providing appropriate referrals and follow-up to ensure service connection. 



  • Conduct home visits and outreach to engage clients, build trust, and provide ongoing support throughout pregnancy and postpartum periods. 



  • Motivate clients to obtain prenatal and postpartum care and reduce health risk factors through education and empowerment. 



  • Provide health education on prenatal care, infant development, nutrition, and other relevant topics to promote healthy lifestyle choices. 



  • Accurately complete and maintain client records, ensuring timely and compliant documentation according to program requirements. 



  • Represent the Perinatal Outreach Program at community events such as health fairs, forums, and outreach initiatives. 



  • Participate in community committees and boards related to maternal and infant health as assigned. 



  • Attend required trainings, including pursuing Community Care Coordinator certification (may require out-of-town and overnight travel). 



  • Attend agency and program meetings as appropriate. 



  • Perform additional duties as assigned by NHA management. 


 


Position Requirements: 



  • High school diploma or GED . 



  • Community Health Worker (CHW) certification preferred; to obtain within one year of employment. 



  • Valid Ohio driver’s license, current auto insurance, and acceptable driving record. 



  • Reliable transportation with the ability to travel extensively within Lucas County. 



  • Strong organizational, verbal, written, and interpersonal communication skills. 



  • Sensitivity and respect for cultural, religious, and ethnic diversity. 



  • Ability to maintain confidentiality and uphold professional ethics. 



  • Pleasant demeanor, emotional maturity, and the ability to work in demanding clinical circumstances. 



  • Patience, understanding, and empathy in client interactions. 



  • Proficient with Microsoft Windows and Microsoft Word. 



  • Ability to work both independently and collaboratively as part of a healthcare team. 



  • Ability to follow written and oral instructions accurately. 



  • Demonstrates initiative, sound judgment, and professionalism. 



  • Maintains a neat and professional appearance.
