Now Hiring: PSH Case Manager Supervisor

The Permanent Supportive Housing Case Manager Supervisor oversees housing and support services for chronically homeless families and individuals, utilizing the Housing First approach to prioritize stable housing without prerequisites. This role ensures the effective delivery of services by managing and supporting case managers, reviewing case plans and notes for quality, and providing clinical insight.

The Supervisor works collaboratively to address barriers to housing stability, including health, behavioral, and family challenges. This position requires a dedicated and service-oriented individual committed to maintaining compliance with all local and federal regulations while promoting the well-being and success of program participants.

Essential Duties and Responsibilities   

  • Responsible for the overall coordination of service needs related to issues resulting in homelessness, housing instability and family problems (i.e. physical/behavioral, health, alcohol/drug use, domestic violence, sexual abuse, lack of income, lack of family support).

  • Completion of a comprehensive bio-psychological assessment covering pertinent details for all family members participating in the PSH program under one household.

  • In conjunction with the Case Manager, engage in the family as a whole to address any outstanding challenges encountered by the family members to include:

    1. Developing the ISP and implementation

    2. Referral/linkages to:

      • Medical and behavioral health care services

      • Substance abuse treatment

      • Parenting classes

      • Family reunification/family therapy

      • Youth support services

  • Provide clinical insight

  • Monitor and track case managers’ engagement with participants.

  • Review case notes through the lens of quality of information captured and accuracy of case notes, clinical lenses.

  • Review, evaluate, and approve participant case plans for quality and effectiveness.

  • Meets individually with case managers to plan and review cases, discuss engagement strategies, and evaluate the effectiveness of the case managers and services.

  • Review and approve all notices issued to participants for accuracy and completion.

  • Analyzes and ensure Housing Stability Plans are updated quarterly and participants progress toward achieving the goals.

  • Ensure that quarterly recertifications are completed and designed software and files are up to date with this information.

  • Engage in Case Conferences with District assigned staff to discuss cases that may need to include but not limited to more intensive case management transfers.

  • All this must be accomplished and provided in compliance with the District of Columbia’s Department of Human Services Standards and regulations of MBI Health Services, LLC and all Federal, State, and Local law applicable to this agreement.

  • Perform other duties as assigned.

Qualifications

  • Master’s degree in social work, mental health counseling, or related field.

  • Must be a licensed clinical social worker or licensed professional counselor, a graduate social worker, or licensed graduated counselor in the District of Columbia can be considered.

  • Have a minimum of two years of experience in case management, mental health services, and direct service delivery.

  • Professional knowledge of the theories, principles, techniques, and practices of social services delivery systems.

  • Two years of professional experience providing counseling and/or case management service to individuals/families experiencing homelessness or other related populations.

  • Excellent oral and written communication skills.

  • Excellent problem-solving skills.

  • Excellent time management organizational skills.

Additional Information

Salary Range: $ 75,000 - $78,500
