As a Pharmacist- Outpatient, Population Health you will serve as an integral member of the interdisciplinary population health team, responsible for optimizing medication use and improving health outcomes for defined patient populations within our value-based care contracts. This role requires a data-driven approach to identify high-risk patients, develop evidence-based interventions, close gaps in care, and ensure cost-effective, high-quality pharmaceutical care across the health system and associated clinics. The pharmacist will serve as a subject matter expert in medication-related quality measures (e.g., Star Ratings) and drive performance in shared savings programs.
Essential Job Functions:
Medication Management and Optimization:
Conduct comprehensive medication management (CMM) and medication therapy management (MTM) services for high-risk patients with complex chronic conditions (e.g., diabetes, hypertension, hyperlipidemia).
Optimize medication regimens, manage polypharmacy, and identify and resolve drug-related problems, including adverse events, non-adherence, and cost barriers, often through standing order agreements.
Perform medication reconciliation during transitions of care to prevent errors and reduce hospital readmissions.
Value-Based Care and Quality Improvement:
Analyze population health data, claims data, and electronic health records (EHR) to identify medication-related trends, pinpoint at-risk patients, and prioritize interventions.
Develop, implement, and evaluate targeted programs and strategies to improve performance on quality metrics related to value-based contracts (e.g., HEDIS and Medicare Part D Star measures).
Participate in quality improvement committees and perform drug utilization reviews (DURs) to promote safe, effective, and cost-efficient medication use across the system.
Collaboration and Education:
Collaborate with physicians, nurses, social workers, and other interdisciplinary team members to develop and execute patient-centered care plans.
Provide expert, evidence-based drug information and education to providers, staff, and patients to improve health literacy and influence prescribing practices.
Utilize patient engagement techniques, including motivational interviewing and telepharmacy, to promote behavior change and medication adherence.
Required Qualifications:
Graduate of an ACPE accredited School of Pharmacy
Doctor of Pharmacy or another advanced degree preferred.
Registered pharmacists in the state of Illinois or eligible for same.
Exceptional aptitude or knowledge may be considered in lieu of experience
Knowledge of contemporary hospital/clinical practice and service.
Strong written and verbal communication skills.
Ability to interact constructively with supervisors, peers, support personnel, and students.
Knowledge and understanding of general hospital operations.
Ability to integrate clinical services, distributive services, education, and research activities
Preferred Qualifications:
Completion of an ASHP accredited residency
Two years of pharmacy experience in an organized health care setting
