Job Title: Pharmacist, PRN

Supervised by: Pharmacy Director

Job Summary: Interprets physician prescriptions and medication orders. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department staff. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.

Demonstrates Competency in the Following Areas:

General Responsibilities:  Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed.

Compounds medications using standard formulas and processes, such as weighing, measuring and mixing ingredients.

Monitors all posting onto the patient   profiles in order that accuracy, drug allergies, drug interactions, duplicate   therapy, rational drug therapy and incompatibilities may be realized and   better patient care received.

Checks patient medication prior to   delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed.

Performs all aspects of patient care   in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.

Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.

Directs Pharmacy Services staff engaged in admixing, packaging and labeling pharmaceuticals.

Answers questions and provides   information to physicians, nurses, patients and other healthcare   professionals on the appropriate use of medications, drug interactions, side   effects, dosage, storage, etc.

Assists in teaching programs (departmental, medical, nursing, community).

Endorses policies and procedures of the department and of the hospital.

Ensures the maintenance and proper operation of equipment and supplies in the work areas.

Maintains established procedures concerning quality control, security of controlled substances and disposal of   hazardous waste.

Instructs students and other medical staff on matters pertaining to Pharmacy Services.

Reviews and monitors drug therapy that requires dosing adjustments according to protocols as authorized by the  physician.

Provides pharmaceutical care to all ages of patients from newborns to the elderly. Understands that there are dosage variations   between newborn, pediatric, adult and geriatric patients, and knows where to   find appropriate information.

Treats patients and their families   with respect and dignity. Identifies   and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.

Assists the Pharmacy Director on special assignments.

Assists the Pharmacy Director with responsibilities in accordance with the organization’s mission, policy and any applicable laws.

Interviews and assists in the hiring   process, trains staff, plans, assigns and directs work, addresses complaints and resolves problems.

Demonstrates ability to read, analyze and interpret pharmacy-related journals.

Demonstrates ability to respond to   common inquiries or complaints from customers, regulatory agencies or members   of the hospital staff.

Demonstrates ability to apply   mathematical operations to such tasks as dosage calculations, formulation of   pharmaceutical preparations and data analysis.

Demonstrates ability to define   problems, collect data, establish facts and draw valid conclusions. Demonstrates ability to interpret an   extensive variety of technical instructions and deal with several abstract   variables.

Maintains accurate controlled   substance records that reflect the perpetual inventory as well as the   ultimate fate of the drug within the hospital.

Compounded Sterile Preparations:

Assures the availability of adequate equipment to safely prepare compounded sterile preparations.

Ensures that pharmaceutical preparations are sterile, properly packaged, labeled and stored, and not outdated.

Certifies the skills of the compounding staff.

Maintains air quality check records.

Ensures compliance with USP 

Medium-risk compounding: The applicable media-fill test will be performed and passed annually, after the initial training and media-fill test is completed and passed.

  

Professional  Requirements:

 

Adheres to dress code, appearance is neat and clean.

 

Completes annual education requirements.

 

Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization   standards.

 

Maintains patient confidentiality at all times.

 

Reports to work on time and as scheduled, completes work within designated time.

 

Wears identification while on duty, uses computerized punch time system correctly.

 

Completes in-services and returns in a timely fashion.

 

Attends annual review and department in-services, as scheduled.

 

Attends required staff meetings annually or reads and returns all staff meeting minutes.

 

Represents the organization in a positive and professional manner.

 

Actively participates in performance improvement and continuous quality improvement (CQI) activities.

 

Complies with all organizational policies regarding ethical business practices.

 

Communicates the mission, ethics and goals of the hospital, as well as the focus statement of the department.

    

Requirements

 Regulatory Requirements:

· Doctor of Pharmacy degree or Bachelor of Science degree or equivalent.

· Graduated from an accredited School of Pharmacy.

· Current Pharmacist certification or licensure in the State of Arkansas.

Language Skills:

· Able to communicate effectively in English, both verbally and in writing.

· Additional languages preferred.

Skills:

· Basic computer knowledge.

· Automated pharmacy systems, IV Laminar Flow Hood or the willingness to learn.

Physical Demands:

· Normal hospital environment. Close eye work. Hearing within normal range. Operates computer. Frequent standing, walking, bending, and stooping. Lifting and carrying up to 35 pounds.

· This position is a safety sensitive position pursuant to Amendment 98 to the Arkansas Constitution § 2(25). A safety sensitive position means any position in which a person performing the position while under the influence of marijuana may constitute a threat to health or safety. This position is therefore “safety sensitive” due to the fact it requires the following: performing life-threatening procedures; working with hazardous or flammable materials, controlled substances, food, or medicine; and a lapse of attention while working in this position could result in injury, illness, or death.
