The Clinical Coordinator of Pharmacy is responsible for the coordination of all clinically related services provided by the Pharmacy Department. All duties are carried out in accordance with administrative policies, applicable national standards and state and federal laws.
This opportunity is full time Monday-Friday
ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinates all clinically related services provided by the Pharmacy Department to the hospital.
Assists the Pharmacy Director in establishing new and improved clinical and distributive services and develop proficiency of the staff pharmacists.
Monitors adverse drug reactions (ADRs) in the hospital, and report this information to the P&T Committee.
Provides routine guidance and assistance to staff pharmacists in the performance of daily clinical functions.
Monitors, when necessary, individual patient’s drug therapy, using dosing calculations and other clinical parameters determine appropriate dosing for those drugs with a narrow therapeutic index.
Will review drug therapy to prevent and detect undesired drug interactions, adverse drug reactions, and contraindications.
Consults with physicians, making patient-specific recommendations when appropriate.
Provides drug information services to other health care professionals.

Requirements
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience
EMR/EPIC experience preferred
Must have 3+ years as a staff pharmacist in acute care
Microsoft Office (Word and Excel) experience preferred
Basic computer and typing skills required
Illinois Pharmacy License.
PharmD, residency preferred
BCPS preferred
30 credit hours of ACPE-certified continuing education, biannually
