Mosaic Pharmacy Service is a “closed door” pharmacy that provides comprehensive pharmacy care to medically complex and vulnerable seniors, all across the country. We provide a patient-focused, pharmacist-driven care model in collaboration with health systems, assisted living facilities and health plans. Our goal is to help our patients feel better about the medications they take every day by simplifying medication regimens, improving adherence, and driving positive outcomes. Job Profile
The Pharmacy Technician - Patient Enrollment Specialist (known internally as the Onboarding Specialist) serves at the front lines of our work with referred patients, completing a unique, virtualized medication assessment and onboarding patients to our unique in-home pharmacy service. Your skills and experience in social work, home health, and/or pharmacy will be invaluable as you receive referrals from our health care organization clients, get to know new patients and their drug therapy challenges, and get them started with Mosaic Pharmacy Service. This job requires compassion, organization, attention to detail, multi-tasking, and top-notch skills in patient communication. Key responsibilities include the following:
Conduct Mosaic patient onboarding interviews, by phone and/or videoconference:
Educate patients about our pharmacy service;
Interview patients about their medications, providers, and health conditions;
Thoroughly and accurately document patient information in our pharmacy management system; and
Synchronize medication regimens and schedule initial medication delivery
Serve as point-of-contact for new patients until delivering first medication delivery
Promptly follow-up with patients to resolve outstanding questions
Complete delegated pharmacy technician acts as instructed by a supervising pharmacist, or other duties as assigned by management
Collaborate effectively with supporting pharmacy technicians to ensure accurate data entry and successful prescription transfers, working with prescribers to obtain new prescriptions, and completing telephonic medication review calls with patients.
Ensure the onboarding process meets company standards and patient expectations
Advocate for continuous improvements in the onboarding process

Requirements
Proof of an active Virginia pharmacy technician license required.
High School diploma or GED equivalent is required.
Minimum of 2-4 years of experience in direct patient communications in settings such as home health, hospital, primary or specialty physician practice, pharmacy or similar. Experience collecting medication histories is a strong plus.
