Physical Therapist

• Assess patients' physical and functional needs at home.


• Develop, implement, and modify individualized treatment plans.


• Educate and guide patients, families, and home health aides.


• Recommend and assist in obtaining necessary assistive devices.


• Coordinate care with PTAs, HHAs, and other healthcare professionals.


• Maintain accurate and timely documentation of patient progress.


• Follow infection control protocols and uphold patient confidentiality.


• Participate in team meetings and performance improvement activities.

• NY State PT License (required)


• 1+ year of physical therapy experience (geriatrics preferred)


• Home care experience (preferred)


• Ability to work independently and travel within Albany County

• Company car provided (for eligible employees)


• Competitive salary: $75K - $87.7K/year (full-time)
