Physical Therapist

Job Description: We are seeking a skilled and compassionate PRN Home Health Physical Therapist to join our dynamic team in the Delray Beach area. As a PRN Physical Therapist, you will provide comprehensive physical therapy services to patients in their homes. This role requires flexibility, excellent communication skills, and a commitment to patient-centered care.

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Provide direct physical therapy services, including therapeutic exercises, manual therapy, and functional training.
• Monitor and evaluate patient progress, modifying treatment plans as necessary.
• Educate patients and their families on treatment goals, home exercise programs, and safety measures.
• Document patient care and progress accurately and timely in compliance with company and regulatory standards.
• Collaborate with multidisciplinary team members to ensure optimal patient care.
• Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in continuing education opportunities.

Qualifications:

• Valid state license to practice as a Physical Therapist.
• Graduation from an accredited Physical Therapy program.
• Minimum of 1 year of experience in home health care (preferred).
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work independently and manage a flexible schedule.
• Valid driver's license and reliable transportation.
