Job Title: Physical Therapist

Location: Cascadia Healthcare

We are seeking a skilled Physical Therapist to join our team at Cascadia Healthcare. As a Physical Therapist, you will be responsible for providing services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents/patients suffering from injuries or disease. 

Responsibilities:

- Develop and implement individualized treatment plans for patients/residents

- Evaluate and document patient progress

- Communicate with healthcare team members regarding patient care

- Educate patients/residents and their families on treatment plans and exercises

- Maintain accurate and up-to-date patient records

Qualifications:

- Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program

- Current Physical Therapist license in the state employed

- Current CPR certification

- Six months experience in a long-term care environment
