About the Role
We are seeking a compassionate and skilled Home Health Physical Therapist to join our dedicated team in Boynton Beach. In this role, you’ll provide personalized physical therapy services to patients in their homes, helping them regain mobility and improve their quality of life. Your expertise will be instrumental in developing and implementing effective treatment plans tailored to each patient’s needs.
Key Responsibilities
- Conduct comprehensive assessments to evaluate patients' physical abilities and limitations
- Develop and execute individualized treatment plans to restore function and mobility
- Educate patients and families on exercises, safety, and injury prevention
- Collaborate with physicians, nurses, and the interdisciplinary care team
- Maintain accurate, timely documentation of patient progress and care plans
- Supervise and guide Physical Therapy Assistants and Home Health Aides as needed
Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license in the State of Florida
- Minimum one year of experience in physical therapy; home health experience preferred
- Experience with OASIS documentation and EMR systems (e.g., HCHB) is a plus
- Current CPR certification
- Strong communication and time management skills
- Reliable transportation, valid driver's license, and automobile insurance
