Serving Deschutes, Crook, and Jefferson counties, our dedicated team of over 200 employees and 200 volunteers delivers compassionate, patient-centered care across a 10,000-square-mile area. Additionally, Hospice House in Bend, Oregon, is a specialty hospital for inpatient hospice care, creating an oasis of tranquility and comfort for patients and their families. 
 

Location: Central Oregon

Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided. 
 

What You Get to Do:  

  • Conduct thorough initial assessments of patients and their families to determine home health needs.

  • Create, monitor, and adjust individualized care plans with measurable goals based on therapy assessments, patient abilities, and diagnoses.

  • Administer appropriate preventative and rehabilitation treatments.

  • Educate patients, families, and caregivers on physical therapy instructions and home exercise programs (HEP).

  • Actively participate in the Fall Intervention Program.

  • Address discharge-planning needs in care plans.

  • Update the Home Health Aide Plan of care as needed.

  • Supervise and evaluate home health aide care every 14 days.

  • Attend staff meetings, case conferences, and IDT sessions as required.

Required Skills & Qualifications: 

  • A degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.

  • Licensed to practice as a physical therapist within the state.

  • Two years of appropriate experience as a physical therapist.  Community/home health experience is preferred.

  • Ability to pass a background test, drug screen, and TB test.  

  • Valid CPR Certification. 

  • Current automobile insurance, valid Oregon driver’s license, and reliable transportation.

  • Strong communication and interpersonal skills.

  • Ability to work independently and collaboratively within an interdisciplinary team.

Compensation: $36.00 - $45.47 per hour
