Physical Therapist

Overview:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will deliver high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients. Working collaboratively with physicians and interdisciplinary team members, you will develop and implement individualized care plans that promote functional improvement, goal attainment, and a positive patient experience across the continuum of care.

Key Responsibilities:

• Conduct comprehensive evaluations and deliver treatment interventions in accordance with legal and ethical standards of practice.

• Assess and document patients’ rehabilitative and functional status; participate in the development and ongoing review of personalized care plans.

• Provide skilled physical therapy services in the patient’s home setting, utilizing evidence-based techniques to support recovery and improved function.

• Educate patients, families, and caregivers on therapeutic exercises, assistive devices, and strategies for continued care outside of therapy sessions.

• Manage daily caseload effectively, including prioritizing patient needs, coordinating care transitions, and collaborating with support staff.

Qualifications:

• Current and valid Physical Therapist license in the state of Florida (Required)

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program

• Basic Life Support (BLS) certification required within 30 days of hire

• Minimum 2 years of recent acute care experience (Required)

• Strong interpersonal, communication, and organizational skills

• Experience in home health care is a plus
