  Evaluate patients and develop individualized treatment plans

  Deliver hands-on physical therapy to support strength, mobility, and recovery

  
  


• 
  

  Set and adjust care goals based on patient progress

  
  


• 
  

  Coordinate with rehab team, nursing, and medical staff

  
  


• 
  

  Lead or direct PT aides/support staff as needed

  
  


• 
  

  Keep documentation current (including prescriptions and signatures)

  
  


• 
  

  Follow safety, infection control, and compliance standards

  Participate in in-services, workshops, and ongoing clinical education

  
  

• 
  

  Doctor of Physical Therapy (DPT)

  
  


• 
  

  Active CA PT license (required)

  At least 1 year as a PT aide preferred, but not required

  Clear communicator and team player
