Position Overview

We're seeking an experienced Physical Therapist to join our premier rehabilitation team at a leading skilled nursing facility in Pawling, NY. In this role, you'll lead personalized rehabilitation programs while working with an innovative, interdisciplinary healthcare team dedicated to exceptional patient outcomes.

Essential Responsibilities

• Design and implement comprehensive rehabilitation programs for diverse patient populations
• Perform detailed patient evaluations and develop evidence-based treatment plans
• Utilize advanced therapeutic techniques to improve mobility, strength, and function
• Lead pain management interventions through manual therapy and therapeutic exercise
• Coordinate care plans with interdisciplinary healthcare teams
• Mentor and supervise Physical Therapy Assistants
• Maintain precise clinical documentation and outcome measurements
• Ensure compliance with all regulatory requirements and safety protocols

Required Qualifications

• Master's or Doctorate in Physical Therapy from an accredited program
• Current New York State Physical Therapy License
• 2+ years of experience in rehabilitation or skilled nursing settings
• Strong clinical reasoning and assessment skills
• Excellence in patient communication and education
• Proficiency in electronic health records systems
