Position Overview

Join a premier rehabilitation and skilled nursing facility as a Senior Physical Therapist in historic Poughkeepsie, NY. In this role, you'll lead personalized rehabilitation programs while working with an innovative, interdisciplinary healthcare team dedicated to improving residents' quality of life and functional independence.

Essential Responsibilities

• Design and implement evidence-based treatment plans for diverse patient populations
• Perform comprehensive physical therapy evaluations and assessments
• Deliver hands-on therapeutic interventions using advanced manual therapy techniques
• Guide patients through therapeutic exercises and functional training programs
• Document patient progress and maintain detailed clinical records
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Mentor and supervise Physical Therapy Assistants
• Ensure compliance with regulatory requirements and best practices

Required Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program
• Current New York State Physical Therapy License
• 2+ years of clinical experience in rehabilitation or skilled nursing
• Strong interpersonal and communication skills
• Proficiency in electronic medical records (EMR) systems
