Physical Therapist - Home Health Care (Full-Time)

We're seeking a compassionate and skilled Physical Therapist to join our patient-centered home health care team in delivering exceptional therapeutic care. As a valued member of our healthcare family, you'll work independently while collaborating with an interdisciplinary team to help patients achieve their rehabilitation goals in the comfort of their homes.

Core Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve patient mobility, strength, and function
• Document patient progress and maintain detailed clinical records
• Educate patients and caregivers on exercise programs, safety measures, and therapeutic techniques
• Collaborate with physicians and healthcare team members to optimize patient outcomes
• Manage caseload efficiently while maintaining high-quality care standards
• Participate in care plan development and implementation

Required Qualifications:

• Current Physical Therapy license in good standing
• Bachelor's degree in Physical Therapy (DPT preferred)
• BCLS certification (or ability to obtain within 30 days)
• Valid driver's license and reliable transportation
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal abilities
