Physical Therapist - Home Health | Full-Time | Waco, TX
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our growing home health care team in Waco, Texas. This full-time position offers an exceptional opportunity to provide personalized, one-on-one patient care in the comfort of patients' homes. As a home health physical therapist, you'll play a vital role in helping patients regain mobility, independence, and improve their overall quality of life through customized rehabilitation programs.
Key Responsibilities
Conduct comprehensive in-home patient assessments and evaluations to identify rehabilitation needs and functional limitations
Develop and implement individualized physical therapy treatment plans tailored to each patient's specific goals and conditions
Provide therapeutic exercises, manual therapy techniques, gait training, and other evidence-based interventions
Educate patients and family members on home exercise programs, fall prevention strategies, and techniques to improve mobility and function
Monitor and document patient progress, making adjustments to treatment plans as needed to optimize outcomes
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated, comprehensive patient care
Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory standards
Manage caseload efficiently while traveling to patient homes throughout the Waco area
Qualifications & Requirements
Current, unrestricted Physical Therapist license in the state of Texas
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Home health care or outpatient rehabilitation experience preferred, but new graduates welcome to apply
Strong clinical assessment and treatment skills across various patient populations
Excellent interpersonal, communication, and patient education abilities
Proven ability to work independently with strong time management and organizational skills
Valid driver's license, reliable transportation, and current auto insurance
CPR certification (or willingness to obtain upon hire)
Compensation & Benefits
Competitive annual salary: $105,000 - $115,000, commensurate with experience
