Job Description

Human Capital Advisors (HCA) is currently seeking a Physical Therapist for one of its clients in Central Pennsylvania.  The main function of the role will be to perform physical therapy in accordance with IFSP per individual child and family. 

Responsibilities

  • Working with children ages birth to three years who qualify for Early Intervention Services and their caregivers/families.
  • Developing the IFSP (Individual Family Service Plan) in coordination with the IFSP Team. This entails developing Objectives and Goals specific to child’s individual needs in one or more of the specified delayed areas.
  • Implementing the IFSP/IEP within 14 calendar days after completion of the IFSP/IEP unless there is adequate documentation that the start date is extended due to family request or family/weather related reasons.
  • Deriving and sharing clinical opinions based on IFSP goal achievement per individual child/family need.
  • Maintaining quarterly progress reports for all children on caseload.
  • Completing additional paperwork as required (including but not limited to Session Notes, Ounce/ECAP, Evaluation Reports, Transition, etc.)
  • Initially or annually assessing each child to determine eligibility in program.
  • Facilitates communication between other team members to ensure the highest quality of care for each family.
  • Develops inservice as requested. Attends inservice and other trainings as required. Completes 24hrs of training each fiscal year relevant to early intervention services child development, community resources or services for children with disabilities. (includes mandatory annual training topics and CPR and First Aid certification)
  • Assists with record keeping to ensure compliance with state and federal regulations. Ensures paperwork requirements are organized properly and delivered to the EIS office in a timely manner, as determined by EIS.
  • Participates as a member of the Early Intervention Specialists team meetings.
  • Ensures licenses and prescriptions for services are maintained consistently to ensure continuity of service for children/families.
  • Performs other duties as assigned.

Qualifications

  • Master’s degree in Physical Therapy. 
  • Licensed and registered in Pennsylvania. 
  • Able to lift/support children birth-3yrs old. 
  • Enrolled in Pennsylvania’s Promise, Pelican, and Ounce Systems (will be completed by employer).
  • Good interpersonal and communication skills necessary. 
  • Must be able to organize multiple job tasks and establish effective working relationship with others.
