Position Summary

We are seeking a PRN Home Health Physical Therapist (PT) to join our growing therapy team. The PT will evaluate and treat patients in the home setting, develop individualized treatment plans, and help patients improve mobility, function, safety, and independence.

This position offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients while making a direct impact on their quality of life.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician orders
• Provide skilled physical therapy interventions to improve strength, balance, mobility, endurance, and safety
• Assess home environments and recommend modifications or adaptive equipment as needed
• Educate patients and caregivers on home exercise programs, mobility strategies, fall prevention, and safety techniques
• Monitor patient progress and modify treatment plans as appropriate
• Complete timely and accurate documentation in accordance with company and regulatory standards
• Collaborate with PTAs, therapists, physicians, nurses, and interdisciplinary healthcare team members
• Participate in care coordination to ensure comprehensive patient-centered care

Qualifications

• Doctorate or Master’s degree from an accredited Physical Therapy program (required)
• Active Kansas Physical Therapist license through the Kansas Board of Healing Arts (KSBHA) (required)
• Minimum 1 year of PT experience preferred; home health experience strongly preferred
• Strong communication, organization, and clinical reasoning skills
• Compassionate, dependable, and self-motivated
• Reliable transportation and valid driver’s license
