PRN Home Health & Therapy is a family-owned, Wisconsin-based company headquartered in Appleton and serving 20 counties across central and eastern Wisconsin. We provide skilled nursing, physical therapy, occupational therapy, and speech therapy services to patients in the comfort of their homes.

You Are a Physical Therapist Who:

• Has great written and verbal communication 


• Enjoys building relationships and trust with patients and families


• Works well independently while also collaborating with a supportive team


• Brings compassion, reliability, and clinical skill to every patient interaction

Job Responsibilities:

• Perform in-home patient evaluations and develop individualized treatment plans


• Deliver evidence-based therapy to improve patient mobility, function, and quality of life


• Collaborate with patients, families, and clinical team members to support care goals


• Accurately document treatments and progress, including discharge summaries


• Supervise and guide Physical Therapist Assistants (PTAs) in treatment plans


• Educate patients and caregivers on safe mobility practices and discharge planning

Requirements:

• Current Physical Therapist (PT) license in the state of Wisconsin


• Valid driver’s license and reliable transportation


• Home health experience preferred but not required—we provide training!
