PRN Home Health & Therapy is a family-owned, Wisconsin-based company headquartered in Appleton and serving 20 counties throughout Central and Eastern Wisconsin. We provide skilled nursing, physical therapy, occupational therapy, and speech therapy services in the comfort of our patients' homes.

What You'll Do

• Complete in-home evaluations and create individualized treatment plans


• Provide evidence-based therapy to improve mobility and function


• Collaborate with patients, families, and the care team


• Document patient progress and outcomes


• Supervise Physical Therapist Assistants (PTAs)


• Educate patients and caregivers on safety and mobility

Qualifications

• Current Wisconsin Physical Therapist license


• Valid driver's license and reliable transportation


• Home health experience preferred, but not required


• Strong communication, compassion, and ability to work independently
